✓ 33 years old, diffuse alopecia (Ludwig II), hair loss started at age 23, with family history of baldness (father NW VI).
✓ Started treatment with Minoxidil 5% 1 month before surgery.
✓ Mean hair size.
✓ Normal follicular units.
✓ % of hair in normal telogen.
This 33-year-old patient presented with diffuse thinning involving the entire scalp.
However, he is an excellent candidate for a hair transplant: his donor area is excellent and his slightly wavy hair is an excellent indication.
In addition, he had started taking Minoxidil 5% a month before undergoing surgery, although the treatment was not very effective for the patient.
We decided to perform the FUT surgery and the patient agreed to completely shave the recipient area, which allowed for a flawless job.
In this first session, we decided to focus on the reconstruction of the frontal and medial area, leaving the vertex intact at this stage.
The surgery took place in June 2017, with a total of 3556 FUT grafts.
As can be seen in the photographs posted below, the regrowth was quite rapid, as by the seventh postoperative month the patient’s change in image was already noticeable.
The patient will undergo a second operation next April.
This second operation will be performed by a combo FUT + FUE in two consecutive days, to cover the vertex area.
We will not hesitate to share pictures and details of his second operation as soon as they are available.

As mentioned in the original post, the patient returned to the clinic last April to undergo a second operation, with the aim of covering the vertex and reinforcing the density of the adjacent areas.
The operation was performed using the Combo FUT + FUE technique, in two consecutive days, thanks to which we were able to extract a total of 3602 grafts (2550 FUT + 1052 FUE).
The patient has recently reached 6 months postoperative and has sent us some photos showing the current result. He is enthusiastic about the regrowth rate and the aesthetic improvement achieved so far.
As a side note, it is interesting to note how lighting conditions greatly influence the perception of density (see the last two photos at 6 months postoperative, taken with flash, with the previous ones, taken in natural light).

Technique: Combo FUT (2550 FUs) + FUE (1052 FUs)
✓ Selection and extraction of follicular units performed with the WAW system, conceived and designed by Dr. Devroye and now widely used in the field of FUE.
It consists of a very precise foot pedal to control the movement of the punch, also designed by the Doctor.
The advantage of the WAW system is that it substantially reduces the number of damaged or transected hairs, thus increasing the quality of the operation.
The system is currently used by a hundred surgeons worldwide.
